How this works

  • Prescribed for youA US-licensed provider reviews your history before anything is filled, and decides whether it's right for you.
  • Filled for your prescriptionMade by a 503A facility for your prescription. Shipped to you.
  • LegitScript certifiedIndependently checked for safe, legal telehealth practice. See our certification
  • Not approved? You pay nothingIf the provider doesn't approve it, it's refunded in full automatically.

What “not FDA-approved” means: compounded medicines are made by a licensed pharmacy and filled for an individual prescription. The FDA doesn't review compounded medicines for safety, effectiveness or quality before they're dispensed, the way it reviews approved drugs. Your provider decides whether one is right for you.

Is this right for you?

It may fit if:

  • You want a licensed provider to review your history before anything is prescribed
  • You want one ingredient, made for your prescription. Made by a 503A facility for your prescription.
  • You want to know who made it and how to report a problem
  • You'd rather not buy something labeled “not for human use”
  • You'd prefer a nasal spray to injections

A licensed provider decides whether it's right for you, after reviewing your intake.

Research

What the research says (and doesn't)

What researchers have studied, and what they have not shown. None of this is a claim about what this peptide does for you; your provider talks with you about what is and isn't known.

Studied

  1. Animal study

A rat study measured brain BDNF (a nerve growth factor) and learning behaviour after a single intranasal dose of Semax.

Source: Dolotov et al., Brain Research 2006

  2. Animal study

A rat stroke-model study measured nerve-growth-factor gene activity after an artery in the brain was permanently blocked.

“the rat brains were analyzed at three time points following a permanent middle cerebral artery occlusion (pMCAO)”

  3. Small human study

A 1997 Russian clinical study gave Semax to 30 acute ischemic stroke patients and compared them with 80 patients on standard care. The abstract does not state randomization.

“Efficiency of Semax (synthetic derivative of ACTH-4-10) was studied in 30 patients”

  4. FDA review

FDA's compiled intranasal human record covered 33 to 47 healthy adults, 69 adults with medical conditions and 451 children. Several sources were meeting abstracts, most did not discuss safety, and FDA found no human pharmacokinetic studies by any route.

“The only ROA discussed in the literature was intranasal.”

  5. FDA review

FDA stated that Semax is a registered drug in Russia, sold as nasal drops, and found insufficient evidence of effectiveness for cerebral ischemia, migraine and trigeminal neuralgia.

“Semax is a registered drug in Russia and is available as 0.1% and 1% nasal drops.”

Not shown

  • The human studies are older, small and mostly Russian, and several exist only as meeting abstracts. FDA found the references it reviewed showed a lack of effectiveness for the uses it evaluated.
  • No human pharmacokinetic data by any route (FDA).
  • FDA raised a possible bleeding risk from reported anti-thrombotic properties, a dopamine finding in mice it called concerning, nasal-spray device concerns and immunogenicity concerns.
  • Registration in Russia is not FDA approval. No ClinicalTrials.gov registration for Semax was found.

Semax is not FDA-approved. In briefing documents for its July 23–24, 2026 Pharmacy Compounding Advisory Committee meeting, FDA proposed not adding Semax to the 503A bulks list; the committee's votes and any final FDA decision were not found. HHS's interim policy describes the conditions under which HHS does not intend to act against a 503A pharmacy that compounds it for a patient's own prescription.

What you should know

What it is

Semax is a 7-amino-acid peptide. It is one of the six peptides in an interim HHS policy for compounding pharmacies. Compounded drugs are not FDA-approved and are not the same as brand-name medications. Nothing is made or shipped until a licensed provider approves it for you. If a provider says it's not right for you, you pay nothing.

Who it's for

You want a peptide prescribed by a licensed provider, not a research-use vial. You want one ingredient, made for your prescription. You want a person to ask between visits.

Can I get Semax with a prescription?

Yes, by prescription. Semax is one of the six peptides in HHS's interim policy (October 2026). A licensed provider decides whether it's right for you; if it is, it's made for your prescription by a 503A facility and shipped to you. Compounded drugs are not FDA-approved and are not the same as brand-name medications.

Do I need a membership for Semax?

No. You can order Semax on its own: a short intake, a licensed provider's approval, and your prescription made and shipped. A membership adds four lab panels and four provider visits a year and a wellness consultant between visits.

How it's made

Made by a 503A facility for your prescription. One ingredient per product. Never blended. It is labeled with your name and your prescription.

What comes in the box

Your Semax Nasal Spray, labeled with your name and prescription, and the pharmacy insert.

Who sets the dose?

Your provider sets the dose.

Is it FDA-approved?

No. Compounded drugs are not FDA-approved and are not the same as brand-name medications. The HHS policy that covers it is not FDA approval, and HHS can revisit it.
