What “not FDA-approved” means: compounded medicines are made by a licensed pharmacy and filled for an individual prescription. The FDA doesn't review compounded medicines for safety, effectiveness or quality before they're dispensed, the way it reviews approved drugs. Your provider decides whether one is right for you.
A licensed provider decides whether it's right for you, after reviewing your intake.
What researchers have studied, and what they have not shown. None of this is a claim about what this peptide does for you; your provider talks with you about what is and isn't known.
FDA described TB-500 as a synthetic, acetylated 7-amino-acid fragment (residues 17 to 23) of the 43-amino-acid protein thymosin β4, and found no published report of TB-500 itself being given to patients for any condition.
“FDA did not find any information in the medical literature where TB-500 was administered to patients to treat any disease”
In a fibroblast scratch-wound assay reviewed by FDA, intact TB-500 showed no activity; one of its breakdown products did. FDA found no in-vivo efficacy studies of TB-500 itself.
“TB-500 (free base; at the concentration of 50 µg/mL) did not induce wound healing in confluent cultures of fibroblasts”
In rats and in vitro, TB-500 was broken down into smaller fragments. One fragment was detected in urine for up to 72 hours.
“the metabolite Ac-LKK was a long-term metabolite of TB-500 detected up to 72 hr.”
A Phase 1 randomized trial gave intravenous thymosin β4 (the full 43-amino-acid protein, not TB-500) to four groups of 10 healthy volunteers, at single doses and 14-day daily doses.
“There were no dose limiting toxicities or serious adverse events.”
A randomized Phase 1/2 trial of TB-500 on cardiovascular biomarkers in stable atherosclerotic disease is registered in China, with 80 people planned and recruiting under way. It has no results yet.
“TB-500 (Thymosin Beta 4 17-23 Fragment) for Cardiovascular Biomarkers in Stable ASCVD”
TB-500 is not FDA-approved. In briefing documents for its July 23–24, 2026 Pharmacy Compounding Advisory Committee meeting, FDA proposed not adding TB-500 to the 503A bulks list; the committee's votes and any final FDA decision were not found. HHS's interim policy describes the conditions under which HHS does not intend to act against a 503A pharmacy that compounds it for a patient's own prescription.

If a provider says it's not right for you, you pay nothing.
